Medical Report

Please refer below for the Medical Report Application Flow.

For patients below 18 years old, the consent of a parent or legally acceptable representative is required.

Submit Claim
The standard Claim form from other institutions must be submitted together with the PAH Consent Form and emailed to [email protected]

Advise the Cost
The Medical Report Officer will advise on the cost of the medical report at the discretion of the doctors.

Remit Payment
Remit payment through a bank or pay at the PAH Cashier Counter. Please reference the patient’s hospital number to avoid delays in processing.

Ready for Collection
The standard medical report or insurance claim will be ready for collection within 14 working days upon payment. There are cases where requests may take longer than the stipulated completion period due to incomplete information.

Receive SMS
An SMS notification will be sent to the registered number upon report completion.
